New Client Intake Form

All new clients should download this form, fill it out completely and bring it to your Rolfing session. If you are unable to print it prior to your first session just be aware that I will have you fill it out at the beginning of the session and this will add about 15 minutes to the overall time. Download the Client Intake Form

Directions From Portland (South):

• Take I-5 North to exit 1B toward city Center/Convention Center
• Merge onto C St
• Destination will be on the right
• Look for porches on the north side of the building; park in available spots

Directions From Vancouver (North):

• Take I-5 South to exit 1C for Washington 501/Mill Plain Boulevard toward City Centre/Port of Vancouver
• Keep right at the fork, follow signs for City Center and merge onto E 15th St
• Turn left onto C St
• Destination will be on the left
• Look for porches on the north side of the building; park in available spots
